For excess with sudden onset, a loud sound and irritability: needle SI19, TE17, GB20, LR3 with reducing method and bloodletting at LR2. For deficiency with gradual onset and a fine sound: needle SI19, TE17, KI3 with tonifying method.
Tinnitus and Deafness: Liver-Gallbladder Fire or Kidney Deficiency
Clear liver fire and open the ear, or tonify the kidney and benefit hearing. Excess is reduced, deficiency is tonified.

RATIONALE
SI19 and TE17 are the local points in front of and behind the ear; GB20 clears the head and the channels reaching the ear; LR3 clears liver fire in the excess pattern, while KI3 opens the kidney to the ear in the deficiency pattern, since the kidney is held to open into the ear. The same local points serve both, and the distal points decide the direction of treatment.
